Hello fellow Uber Partners!
We’ve been getting a lot of questions about the Uber SPA Forms as well as requests for a copy of the Uber SPA Forms. So, we decided to post this Uber SPA 101 to help you with your TNVS requirements submission.
Why do I need to submit these Uber SPA Forms?
As a new Uber Partner, you’re required to upload the Uber SPA Form/s before Uber will activate your partner account. It’s very important to submit the SPA/s because you won’t be able to get activated and receive requests if you don’t submit.
From the vehicle, driver requirements and SPA/s you submitted, Uber will use these to apply for the Provisional Authority (PA) to LTFRB on your behalf.
There are 2 SPAs, which SPA do I need to submit?
For your information guys, there are two (2) SPAs.
- SPA A: SPA with Uber (Required for all TNVS Applicants)
- SPA B: SPA with Vehicle Owner (if registered Uber Partner is NOT the vehicle owner)
TIPS: Quick guide to know which SPA you need to submit (refer to the infographic):
SCENARIO 1: If the Vehicle Owner will be the one to directly register as the Uber Partner, then, you only need to submit ONE (1) SPA which is SPA A: SPA with Uber. This SPA is to authorize Uber and its legal counsel to represent you for TNVS accreditation.
SCENARIO 2: If the Uber Registered partner is NOT the Vehicle Owner, you need to submit TWO (2) SPAs. Example, the car you’ll register to Uber is owned by a friend or kamag-anak but someone else will register as a Partner. In this scenario, the Vehicle Owner needs to execute 2 SPAs— one is to authorize Uber and its legal counsel to represent you for TNVS accreditation (SPA A) and the second one to authorize the Non-Vehicle Uber Registered Partner (SPA B) to represent you in all the transactions related to Uber.

SPA Requirements for Scenario 1
These requirements are for Uber Registered Partner who OWNS the vehicle.
REMEMBER: Should be signed and notarized by the VEHICLE OWNER.
SPA Requirements for Scenario 2
These requirements are for Uber Registered Partners who DO NOT own the vehicle. (ex. The car is owned by spouse, kamag-anak, tito, tita, lolo, lola, kaibigan)
REMEMBER: All these should be signed and notarized by the VEHICLE OWNER.

Dear Uber,
Ask ko lang po kc nagpasa na po ako ng SPA nong Jan. or February. Ang application ko po ay nong pang nov. 2015, ask ko lang kung magpapasa ba ulit ako ng SPA A na ito since submitted na po yong SPA signed and notarized copy sometime jan or feb. 2016. Thank you.

Tanong ko lang po registered partner driver na po ako ang sasakyang gamit ko po ay Innova 2016 plate no./CS YV9647. sa ngayon po ay bumili ang anak ko ng sasakyan APV SUZUKI at ito na po ang aking gagamitin. ano-ano ang requirements na dapat kung isubmit paki tulungan po ako para maihanda ang mga requirements. I jusy want to clarrify if uber is still accepting partners and drivers at this day oct 24. Since there rumors that it had stopped and if there is a hotline that i can reach for inquiries. Thank you.
Yes, Uber still accepts sign ups. Uber will activate you within 24-48 hours if you submit the required docs. Submit the requirements so when LTFRB resumes the applications, maipapasa agad ni Uber yung requirements ninyo.

Hi, I’m an uber driver. Just want to know if updated ba yung SPA form niyo for TNVS application with Pricewaterhousecoopers Buisness Services Philippines Co. Ltd., Atty. Cherry Ongson as the designated attorney-in-fact? Or should it be Quisumbi Torres Lawfirm with Attys. Calimon, Lazaro and Aguila? Or they are all okay depending on the location for application because Pricewaterhousecoopers is at Pasig City and Quisumbing is at Global Taguig? Need answer ASAP. Thanks!
For your Partner/Driver concern, we recommend you report the concern via:
1. Uber Driver App. Select the Trip > Help > Select the issue
2. Uber Help Center. Go to https://help.uber.com, select city as Manila and choose “For Partners”. Sign in with your Partner/Driver account and select your issue/concern.
3. You can also go to Uber’s Office in The Square, Greenfields, Mandaluyong to have your issue fixed personally by an Uber support staff. Operating time is 9am-5pm, here’s the schedule of Partner Activation Centers: http://driveonuber.com/s chedules/
